We are an independent geriatric care management service established to assist older or disabled adults and their families in realistically assessing medical, emotional, social and practical day-to-day care management needs. We develop and oversee personalized care plans that allow our clients to live as fully and independently as possible.
We are able to simplify and streamline the process of identifying the needs of each client, securing the resources best able to meet those needs, and coordinating the provision of services.
We have gained extensive experience through several years of helping clients and families with the issues of aging, Alzheimers and other dementias, mental health problems, chronic medical conditions and the intergenerational issues families face.
Our case managers are experienced professionals with degrees in nursing and advanced degrees in social work, special education and divinity. We adhere to the Standards of Practice and the Code of Ethics of the National Association of Geriatric Care Managers.
